RN Hospice Case Manager – Salt Lake County, UT

Envision Home Health & Hospice has an opening for a full-time RN Hospice Case Manager.

We are looking for a skilled, experienced nurse who is passionate about providing quality end of life care to patients and families.

The RN Case Manager is responsible for coordinating all aspects of the interdisciplinary care and services provided to hospice patients and their families. This position requires strong general nursing skills including nursing assessment, symptom management, patient/caregiver teaching, coordination of services, collaboration with other professionals, documentation of care, and promoting comfort and quality of life on a daily basis. Excellent communication skills are required.

RN On-Call Hospice Nurse – Salt Lake County, UT

Are you looking for a great 2nd job for extra income that also provides some flexibility?
This position provides On Call coverage 1-2 week nights per week from 1700 to the next morning at 0830 and 1-2 weekends per month from Friday at 1700 to Monday morning at 0830. The days and weekends that you work can be customized to fit your schedule.

Responsibilities include:
Make visits to hospice patients as needed/requested after hours.

Answer incoming after hour phone calls from hospice patients or caregivers.

Admission of new patients after hours.

Job Requirements:

  • Current RN license to practice in the state of Utah.
  • Current Utah driver’s license
  • Reliable transportation
  • Maintain current CPR certification

Responsibilities and Duties

Job Responsibilities

  • Performing personal care and assistance with Activities of Daily Living (ADL’s) such as bathing, dressing, toileting, transferring (walking) and incontinent care. May perform light housekeeping such as keeping patient’s living area clean and orderly, changing sheets and making their bed. Providing patient and family with positive communication techniques, within the realm of ethical and respectful care, including confidentiality.
  • Meeting safety needs of the patient and using equipment safely and properly (foot stools, side rails, O2, etc.)
  • Reporting on patient’s condition and significant changes to the RN Case Manager.
  • Adhering to the organization’s documentation and care procedures and standards of personal and professional conduct

Qualifications and Skills

Physical Requirements

  • Ongoing need for the employee to see and read information, identify patient needs, identify equipment and supplies.
  • Frequent interactions with patient care providers, patients, and visitors that require the employee to verbally communicate as well as hear and understand spoken information.
  • Expected to lift and utilize a full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.
  • Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment.

Minimum Requirements

  • Current Utah Certified Nursing Assistant Certificate (CNA).
  • Current driver’s license, auto insurance, reliable transportation.
  • CPR/First Aid Certified
  • Must be at least 18 years old.
  • Ability to transfer and position patients independently and safely.
  • Effective interpersonal communication skills.

Hospice Director of Clinical Services – Utah County, UT
$3K sign-on bonus

Responsible for the delivery, coordination, and supervision of Hospice services provided to patients and families in all settings consistent with Hospice policies and procedures and best practices in end of life care. Hires, trains and mentors hospice clinicians. Develops standards that ensure safe and effective services to patients and families. Monitors caseloads, oversees care coordination and provides leadership to hospice team.

In addition to having a current RN license from the State of Utah, candidates must have the ability to work independently, make accurate – and at times – quick judgments, supervise others, and respond appropriately to a crisis.

2 years of experience in hospice management preferred.
