Who qualifies for hospice care?
Hospice care is available to anyone with a life-limiting illness including (but not limited to) cancer, heart, lung, neurological, kidney, liver, end-stage diabetes, Alzheimer’s, and Parkinson’s diseases. Other indications qualifying a need for hospice may include uncontrolled weight loss, significant decline in function, or failure to thrive.

Where can I receive hospice care?
Hospice care can come to you at home, in assisted living centers, retirement centers, and skilled nursing facilities.

Will my insurance cover hospice services?
Envision services are covered by Medicare and many private health insurance companies. Our staff can assist you in determining your insurance coverage and payment options.

Who is part of the hospice team?
The hospice team is comprised of physicians, nurses, social workers, therapists, nursing assistants, chaplains, equipment suppliers, and volunteers.
