Your doctor will direct the care provided by Envision staff. While your doctor will establish the frequency of your care, Envision healthcare professionals will work with your schedule to make the timing as convenient as possible.

Will my insurance cover home health services?
Envision services are covered by

Medicare and many private
health insurance companies. Our staff can assist you in determining your insurance coverage and payment options.

Do I qualify for home health services?

	•	Are you restricted in your ability to leave your home?
	•	Are you a resident of an assisted living facility?
	•	Do you have any healthcare need that could be provided
		by a nurse?
	•	Do you have difficulty walking, standing, or getting in
		and out of the shower or tub?
	•	Do you have difficulty dressing, bathing, or grooming
		yourself ?
	•	Are you over 65 years old?

If you answered “yes” to any of these questions, you may
qualify for home health services.
